In order to attend to you promptly, we request that our patients bring a list of their current medications and insurance cards for each visit. Insurance cards are verified at every appointment. If you are currently on medication please bring your medications and/or medication list with you.
If you have an HMO it is your responsibility to have a referral from your primary doctor. If you do not have one we may have to reschedule your appointment.
Prescriptions
All prescriptions require a 48 hour notice.
Please contact your pharmacy for prescription refills, the pharmacy will contact our office for validation.
Forms
A $10 fee will be charged for forms to be filled out by the physician. This fee must be paid when the form is submitted to the office.
Examples of these forms are: FMLA forms, driver license forms, forms regarding bill payment due to disability. If you have questions regarding a form, please contact the office.
Forms will be completed within 48-72 hours.
All patient sections on the form must be completed prior to submitting them to our office.
Patient Medical Records
Patients will be provided with one copy of their medical record at no cost after signing a records release. If additional copies are requested by the patient there will be a cost which will be determined by the amount of records to be copied.
Medical records requested by other physicians will be provided at no charge with a signed records release.
Medical records requested by an attorney’s office must have a signed release and are subject to prepayment.
